Every work we do requires an RX Form.
This form contains the key information we exchange with the dentists.

Communication is a key component of a successful cooperation with the dentists and here at Celadon Dental Studio we believe it to be the key factor of a satisfied client.
As Michael Slater stated in his presentation at Dental Partners 5th Anual Dentists’ Conference, the successful treatment planning starts with a good communication with the dentists.

Local offices within 5-10 miles radius will be picked up or delivered with our local courier services or by me personally.
Please, feel free to discuss any cases with me at time of pick-up.
Outside areas/out of country , cases will be picked up or delivered by one of our other services:
